The courtroom is available during business hours Monday through Friday. The Judge can also officiate the wedding ceremony at another venue on weekends.

Names of the 2 parties getting married.

Contact person name & telephone number.

Requested date/time of wedding.

Marriage license issued date/time and County issued by.

Number of witnesses to attend.
