Forms to complete if you are working an election
Please complete the forms in their entirety and return them to the Elections Department. In addition, a copy of your social security card is required. The completion of these forms allows us to make sure you are in our payroll system. If you fail to complete them, it will result in the DELAY of pay.
- W-4 Employee's Withholding Allowance Certificate
- Payroll Direct Deposit Authorization Form (include a voided check stub)
